To schedule an appointment, please call the office phone number and talk with one of the staff. It is recommended you have your insurance card at the time of the phone call if you intend to use insurance. Materials you will need to bring to the appointment are a current Driver’s License, or comparable government photo ID, along with your insurance card (if applicable). The doctor or therapist may request further information from the legal custodial guardian if the client is under the age of 18 and legally considered a minor. Methods of payment include cash, credit card, or check, and are due upon service.
